With everyone’s focus on improving care while reducing healthcare costs, much time and study has been spent looking at ways to reduce rehospitalization and emergency room care. What comes to the forefront time and time again is the need for improvement in care transitions. Whether in the hospital, skilled nursing facility, home health or other post-acute care setting, the need for smoother and more efficient care transition policies is a must!
